Hello and welcome! If you would like to review these documents before your first lesson, you will be ahead of the game!
You will be given an individual folder with all the documents below, so no worries if you don’t print them ahead of time. The only form needed before we get to work at your first lesson is the consent and waiver. This can be filled out electronically when you book through Acuity, or you can print it ahead of time and bring to your lesson.
